Sunrise Community Health

Non-profit federally qualified health center running 14 clinics in northern Colorado, providing integrated medical, dental, behavioral and vision care to over 43,000 patients a year.

Sunrise Community Health is a non-profit, federally qualified health center serving Weld, Larimer and surrounding counties in northern Colorado. Founded in 1973 as a migrant health clinic, it operates 14 clinic locations across Greeley, Evans, Windsor and Loveland, and employs nearly 400 people. The organisation serves more than 43,000 patients a year, nine out of ten of whom live at or below 200% of the Federal Poverty Level.

The centre delivers integrated, team-based care through a single model that combines primary medical care, dental, behavioral health and vision services with on-site laboratory, radiology, pharmacy, social work, care management and health education. It also runs the Women, Infants, and Children (WIC) nutrition programme. The organisation has been recognised as a state and national leader in quality care and in the adoption of health information technology, including electronic health records.

Sunrise Community Health describes its mission as providing affordable access to quality health care for all, with a particular commitment to low-income, uninsured and underinsured individuals and families, and to the migrant populations it was originally established to serve.
